Care Manager
Judge Healthcare is currently hiring a Care Manager - Long Term Support Services in Manchester, NH.
This job will have the following
Responsibilities:
Identify scope of long term support services coveredConduct clinical assessments for members who have LTSS needsArrange, coordinate, evaluate, and advocate for services that meet the specific member's needsQualifications & Requirements:
Masters Degree in Social Work, Nursing, Psychology, Education, Public Health, or a related field required.
8 years' experience in direct care servicesA focus on direct care and administrative responsibilities related to long term services/supportsExperience working with a Medicaid populationExperience with a Health Plan+Keywords:
RN, registered nurse, LSW, licensed social worker, LTSS, long term support services, social work, nursingInterested candidates send most current resume to Carlos at email removed .
Estimated Salary: $20 to $28 per hour based on qualifications.
