POSITION
Summary:
Provides patient care activities through the nursing process including, but not limited to, assessment, planning care, implementing physician orders, evaluating patient responses to nursing intervention, revising plans as indicated and maintaining a therapeutic environment.
DUTIES AND
Responsibilities:
Provides advanced nursing care in physician practices with particular emphasis on patient and educational activities. Provides patient/family-learning experiences based on identified needs or projected needs.
Provides advanced patient care with minimal guidance or assistance. Uses and maintains appropriate resources to guide practice. Provides advanced comfort interventions using competent nursing actions.
Manages advanced telephone triage for patients with health problems and utilizes appropriate resources to guide practice.
Collaborates with physicians and staff as needed to provide comprehensive health care to the patient and to reinforce physician s instructions.
Documents nursing actions and patient s responses with evidence of advanced clinical and technical competence in advanced situations. Uses advanced EMR features to document nursing care. Uses other Epic applications competently. Mentors colleagues to use EMR functions more efficiently.
Modifies the plan of care to meet health care needs of the patient and family.
Acts as a clinical resource and mentor to other clinical and clerical staff. Shares clinical expertise with colleagues.
Actively supports the vision, philosophy, mission and goals and objectives of the organization and PPS, and interacts with other individuals, groups or departments in bringing the vision to reality.
Utilizes the roles and responsibilities of all the intra-office team members efficiently and effectively. Contributes to the overall work assignments equitably. Coordinates care with other disciplines. Promotes teamwork within the entire EPN Network. Promotes team communication.
Develops a plan of care to deliver high quality, cost effective nursing services, utilizing the concepts of the nursing process independently.
Interacts with patients and families regarding clinical complaints for effective and immediate response and resolution. Seeks methods to improve communication with patients/family members. Implements patient satisfaction improvement processes.
Experience:
Telephone triageexperience preferred. Previous office nursing preferred.
Licensure/Certification:
Current registration as a registered nurse in the State of New Hampshire or a multi-state RN License issued by a compact state. Current activeBLS authorization.
