Summary: PRIMARY PURPOSE
Coordinates and delivers quality nursing/health care programs and services to clients/patients of all ages and families whose needs range from basic to complex and are rapidly changing by utilizing the nursing process. In collaboration with the Health Educator for EHS, provides community outreach and educational programs which focus on the community needs assessment and subsequent plan for health promotion and wellness.
DUTIES AND RESPONSIBILITIES
Under the general direction of the Vice President, Home Health/Long Term Care & Community Services, and the direct supervision of the Director, Community Services and within established policies and procedures, the incumbent performs the following functions:
Essential Functions:
- if !supportLists ->1.- endif ->Plans for, coordinates, and provides health screenings, health & immunization clinics, programs and services with a focusing on wellness and disease prevention, providing education, referrals, and follow-up as indicated and appropriate.
- if !supportLists ->2.- endif ->Provides quality nursing care while promoting client/patient dignity and providing a calm, supportive and therapeutic environment for client/patients and their families.
- if !supportLists ->3.- endif ->Confers with local health care practices, and clinics, as a means of community resource networking. Identifies unmet healthcare needs and areas to provide necessary resources.
Graduate of an accredited school of nursing; BSN or equivalent preferred.
Experience:Equivalent of 2 years in a staff nurse position in acute care setting. Prior Public/Community Health Nursing Experience desirable.
Licensure/Certification:Current Licensure in State of New Hampshire as a professional nurse.
Valid Driver s License: Required to travel throughout southern New Hampshire to provide home health care service and other job-related activities.
Other:Proof of auto mobile insurance required.
Motor vehicle background report will be conductedEstimated Salary: $20 to $28 per hour based on qualifications.
