Under the general direction of the Director and in accordance with: standards of care and practice established by the Nursing Organization and, the specific department; the rules, regulations and/or standards established by the State Board of Nursing, the Joint Commission for Accreditation of Healthcare Organizations, and appropriate specialty organizations relevant to the patient population under care, the incumbent performs the following functions:
Essential Functions
Patient Care
Assessment:
Interviews and examines the patient and/or interviews family to obtain a comprehensive history and to obtain data about the patient's development status, behavior, cultural, spiritual, and health concerns.
Reviews clinical data from diagnostic studies and previous records.
Based on assessment, identifies and documents patient care problems and areas requiring intervention.
Collaborates with the multidisciplinary team to address patient needs.
Reassesses patient status at established time frames and as indicated by the patient's status.
Reports status and changes in status as appropriate.
Planning:
Develops an individualized approach to deliver family-centered nursing care to women and newborns to include patient rights, age-specific developmental needs, physical ability, ethical and cultural considerations, and incorporates input and preferences whenever possible.
Establishes desired goals with the patient, family, and multidisciplinary team.
Initiates discharge planning as an integral part of the plan of care at the time of admission and updates status of the discharge plan on a continuing basis.
Documents plan of care appropriately per policy of the nursing organization.
Develops plan of care to support The Mom's Place Mission , Vision and the principles of single-room perinatal care
Implementing Plan of Care:
Coordinates and incorporates diagnostic and therapeutic orders. to implement nursing care.
Provides for patient's dignity during provision of care.
Assures calm, supportive and therapeutic environment for patients and their families,
serving as an advocate when appropriate.
Assesses the patient's response to and understanding of care provided and reports this as indicated.
Provides education based on patient/family's assessed needs.
Upon completion of orientation/cross-training, the incumbent will maintain competency and function in each of the following areas: mother/newborn care, low-risk antepartum care, intrapartum care, circulator and recovery role for cesarean birth, and women's surgical care within the job description.
Evaluating Plan of Care:
Reviews and revises the plan of care in collaboration with the patient/family and other multidisciplinary team members.
Documents progress toward expected patient outcomes.
Reassesses patients' abilities, and documents and communicates continued patient needs into the plan of care.
Qualifications:
Education
Graduate of an accredited school of nursing, BSN preferred. Obtain Certification in Neonatal Resuscitation Program within 3 months of hire. Attend a Basic Fetal Monitoring Course within 6 months of hire. Participates in Perinatal skills training as directed by clinical leadership.
Experience
A graduate nurse with perinatal experience preferred.
Licensure/ Certification
Current registration in the State of
new hampshire
as a Registered Nurse
