ServiceLink Caregiver Support Counselor

ServiceLink is looking for a Caregiver Support Counselor to join our team! The individual in this key position will provide person-centered education, advocacy, assistance and supported referrals to help NH residents retain their dignity, quality of life and highest level of independence within their community.
Responsibilities include Compliance with all contract requirements pertaining to the NH Family Caregiver Support Program and Long-Term Support Counseling Work closely with family and/or relative caregivers including grandparents Complete training through the Bureau of Elderly and Adult Services designee.
Complete mandated assessments on caregivers who are referred to the caregiver program.
Maintain bimonthly contact with all caregivers under the Caregiver program as required by the Servicelink Contract.
Perform six formal caregiver outreaches per contract requirements.
Maintain current knowledge of Family Caregiver Support Program services, policies and procedures.
Perform person-centered assessment and counseling for referrals to long-term services and support through telephone, walks-ins, emails and multi-media.
Completes eligibility screening and provides information to assist individuals in understanding and applying for State and Federal program.
Completes documentation on all contacts using protocols/processes as mandated by the contract.
Complete follow-ups on received contacts to determine if needs were met.
Assist with all aspects of the Medicare program especially during the Medicare Annual Enrollment Period.
Attends meetings as mandated by contract or required by State.
Obtain all mandatory certification as required by the contract which includes the Alliance for Information and Referral Certification (AIRS); State Health Insurance Assistance Program (SHIP) Counselor and Senior Medicare Patrol (SMP) within six (6) months to one (1) year of hire.
Qualifications Bachelor's Degree in Social Work, Education, Human Services, Sociology, Cultural Anthropology, or Behavioral Science, OR Eight (8) years' experience working directly with programs supporting the elderly or disabled population in lieu of degree Basic knowledge of community long-term care support services for the elderly and disabled population, OR Combination of education and experience totaling eight (8) years Basic knowledge of community services such as home health agencies, assisted livings, nursing homes, and hospitals Strong oral, written and interpersonal skills Analytical and problem-solving ability Comfortable with performing home visits and community outreaches Basic level of computer skills to include Microsoft Office application suite preferred Ability to work a flexible schedule, as outlined in ServiceLink contract Valid driver's license and reliable transportation Hours:
